Invitation to Tender – Community Webs

Bristol Ageing Better has collaboration at its heart and we are committed to trying to work with other organisations to develop ideas, test models and to jointly consider sustainability.
One of our original projects under the ‘Identify and Inform’ work stream was called GP Case Finding which had the main aim of identifying Isolated and Lonely Older patients through surgeries and connecting them to the Community Navigators.
Bristol Ageing Better has been working with Bristol Council and Bristol Clinical Commissioning Group across all of our projects and this year we have started to see opportunities to collaborate through their Test and Learn projects, including one called Community Webs.
In a survey by Citizens Advice Bureau in 2015, it was suggested that 19% of GPs' time is spent addressing non-medical issues. This figure fits with Bristol estimates, and while increasing numbers of GPs are engaging with Social Prescribing, most do not have the resources and information about activities and services of a non-medical nature.
The Community Webs model aims to work with those patients with non-medical issues and connect them to activities or services that can address them. They will also receive support that may reduce repeat non-medical visits to surgeries. It is believed that a significant number of these non-medical issues are related to loneliness or isolation and that many of the patients will be over 50.
We have now come together through Better Care Bristol to commission a test and learn project that will deliver the Community Webs model in Bedminster and North Bristol and we are inviting tenders to coordinate the Community Web project and to provide a worker in each area. This project will be delivered in partnership with the CCG and local Primary Care providers and provides a fantastic example of working together to solve common issues.
