Social Prescribing

“Social prescribing provides a pathway to refer clients to non-clinical services, linking clients to support from within the community to promote their wellbeing, to encourage social inclusion, to promote self-care where appropriate and to build resilience within the community and for the individual.” (Social prescribing in Bristol Working Group, 2012.)

If Primary Care professionals think that a patient could improve their health or wellbeing through a non-clinical activity then they provide them with a “social prescription”. This often means that the patients meet a Social Prescribing Coordinator who can connect them to a range of services that are on offer, from weight management classes and 'Kitchens on Prescription' to coffee mornings, walking groups or advice.

There already a range of Social Prescribing projects in Bristol, and BAB is currently working with them to identify how this project may support them to provide better support for those older people who are isolated and lonely.
